Employer documents for your residence application: Annex 1

A practical coordination guide for employees and HR preparing the employment information for a Polish residence case.

The employer annex is part of your residence evidence, not a substitute for the whole application. Its job is to communicate employment information in the required form. The details should match the job you actually intend to perform.

Choose the right person at the employer

Ask who can provide the employment data and who is authorised to sign. Those may be different people. A recruiter may know the proposed role but not be the person responsible for the formal annex.

For covered electronic applications, the MOS process sends the relevant annex link to the employer contact. The required electronic signature must be arranged. A scan of an earlier paper document should not be assumed to complete the current online workflow.

Agree the information before signing

Prepare a short comparison sheet:

Field Check against
Employer identity The legal entity employing you, not only its trading name
Role and duties The actual position and supporting employment documents
Contract type and period The agreed terms, including proposed dates
Pay and working time The contract and any relevant immigration requirements
Applicant details Your identity information in the residence application

This is a coordination aid, not a replacement annex or an instruction to invent missing information. If an item is uncertain, resolve it with HR rather than copying a colleague’s data.

New application or amendment?

The procedure matters. An application to amend an existing residence-and-work permit has a current prescribed form and should not be confused with submitting a new residence case. Start with the correct procedure before deciding which attachment or signing workflow to use. The MOS FAQ addresses the electronic system.

Keep a controlled final version

Mark drafts clearly and agree which version is final. If pay, dates or the employer changes after preparation, notify the person handling the residence case. An annex signed earlier can become inconsistent with later evidence even though the signature itself is valid.

Keep a record of the request, responsible contact and follow-up. It is particularly useful when HR, payroll and management each hold a different part of the information.

How the employee and STAYLEGAL fit in

You provide accurate personal information and tell us about changes. Your employer confirms its employment information. STAYLEGAL coordinates the requests and checks consistency across the file; it does not sign the employer’s declarations on the employer’s behalf without a proper basis.
